Look at US Contract Manufacturers there are some CMs with PCB Fab and/or Assembly capabilities.
Sonic
One of our clients has had good success working with Sonic for PCB fab. They've been very supportive, even with low- to mid-volume medical products. I haven't been working with them directly, so maybe someone else on the list has more direct experience.

We recently had Absolute EMS do some quick-turn PCAs (mix of SMT and PTH) for us over Xmas. They are in Santa Clara, close to SJC. They did a quick flawless job, including sourcing the PCB fab (from Advanced Circuits). Not a super fancy-pants place, but they have a brand new state of the art SMT line from Hanwa. They do Mil and Medical work as well as commercial work.

Sonic Mfg in Fremont has been great to work with and pretty quick. I've worked with them for a few years. There are always the little bugs and occasional quality issues that come up in PCBA but they have done great rework and been really responsive to doing the right thing, even on tiny projects.
They source the PCBs and do a lot of medical work. They have FDA registration and maybe even ISO 13485

There is a really awesome new company known as Circuit Hub (https://circuithub.com/).
I haven't personally used them, but I have heard good reviews.
I've used Screaming Circuits up in Oregon for only 2 PCBS, but it looks like they can do up to 5,000. I remember them being much cheaper than some other assembly houses.

We used to use Lee Mah Electronics a lot. They can do small and large runs. They have a facility in Brisbane and one somewhere in China, can't remember exactly. Anyway they do great work and will build here or in China as per your request.

The part I wanted to explain in more detail is if you want the lowest cost, you take on the sourcing of the components and deliver the "kits" to the assembler. If you have them turnkey the whole thing they need to upcharge for the components to cover the cost of the person that spends the time placing the orders and managing the kit.
I add 25% to BOMs I order for customers.
Places like Digikey have tools for you to upload, save and manage your BOM. Then when it's time to make a kit you set the kit quantity and it'll do the math and spit out a quote / cart for you to purchase. You can also have it drop shipped to you assembler so you never have to touch it.
