Applied Engineering in San Jose.
We used them when I was at eatsa for a number of builds and were really happy with them.
Quantities in the 50-100 piece range, a dozen sheet metal parts, few other custom mechanicals, bearings, PCBAs, pneumatics.

Recommended by PD Alum Robert Miro: They do primarily electrical test systems for chip manufacturers however they also have a good group doing medical device assembly and test work. They would be in close proximity to you in SF and have the capabilities.

Recommended by PD Alum Robert Miro: They acquired a group called Sieger Engineering about 5 years ago. Sieger used to be our go-to resource for complex machining and sub-assembly work. Very high quality with great work ethic.

Initial query to the group for a US-based contract manufacturer to handle:
- Injection Tooling
- Manufacturing (more than a few thousand pieces per PO)
- Packaging (simple die-cut printed paper package card)
- Assembly of the product onto the packaging
- Cartoning for shipment

[Two people recommended Phillips Plastics]
We have had a great experience with Phillips Plastics:
http://www.phillipsplastics.com/capabilities
They mold our parts, assemble our devices on a semi-automated line using these parts and other components from another vendor, and package and ship the final product. We started with low volume production (single-cavity mold) and have painlessly scaled up (just validated our 8-cavity). They are based in beautiful Wisconsin but they have a design center in Sunnyvale. Here is our semi-local rep:

I have used Advanced Engineering and Molding Technology in Riverside, CA. They've been a good vendor. They mold in the U.S. and do assembly operations in Mexico, immediately across the border. Tell them I [Ramon Alarcon] sent you . The contact info is:

They don't do EE assembly, but everything else.
High quality, very knowledgeable. Not the cheapest but worth every
penny.

I have used Pre Plastics in Auburn several times. They use aluminum tooling to keep the tooling costs down for small runs. In general, they are knowledgeable and consistent.
