We use Burns Transcription.  They charge $140/hr (i believe).  Turn around is about a week.  Quality is good.

In the past we used RapidTex, which was purchased by People Support, whose website is:

We used to use RapidText when I worked at Jump. Thye were fine - not the best, not the worst. I recall them being about $60 per recorded hour.

Here is someone who can do transcription, and she is fairly familiar with technology words. That turns out to be a big help!

Gayle Fluster is (was) $250 a two hour focus group tape around 2005. She know has her own company. Here is her contact info:

(817) 329 3567 and gayle@alphabuzzgroup.com

 From my wife:

www.escriptionist.com <http://www.escriptionist.com/>  is by far the most

reasonably priced service.  If 100% accuracy is very important, other

services might be better but pricier.  E.g., Tigerfish is a great SF based

service.
